Select the position(s) that you hold in your current occupation, along with the levels that position applies (check all that apply). As you make your selections, we will construct the appropriate title in the box below. If you do not see the position listed you can select Other and then customize the final title.

School

Search by name for the school. You can leave this blank if: you are a district personnel, you cannot find the school, or you are not affiliated with a school.

Retired Members: If you have retired, please select the desired school/district you retired from.

Important: Please provide information about your tenure in the OADA prior to retirement. This is used to prove that you should receive retirement status.

If unsure, approximate year is acceptable.
Number of years since between first year and this year not spent in position. Used to determine total number of years served.
